Record what you can see

Describe an observation without diagnosing it. A mark on a wall, uneven transition or missing selection can be photographed and referenced. Ask the contractor whether an item is complete, excluded, pending or in need of correction. Do not open equipment, remove covers or enter unsafe spaces to fill out the record.

Agree what happens next

A useful note has an owner and an action, not just a complaint. Keep the contract and scope beside the list and record written clarifications. The worksheet does not decide payment rights, acceptance, warranty coverage or whether work complies with requirements. Ask the relevant professional or advisor where those questions arise.

What the output means

This is a personal observation record, not a professional inspection report or legal acceptance document.
